Advance Directives empower an individual to share their voice with others regarding future healthcare decisions in case of an emergency, where they become debilitated and cannot make decisions about their own health care.
Our unique My Care Journey design guides members through the essential stages of creating their own personal health-related decision documents.
- Members select each directive (Medical Power of Attorney, Living Will, Mental Health and Memorial Planning) they want to create and then provide their answers accordingly.
- Upon completion of the journey, members are directed to save and print for notarizing, then securely store on My Care Voice.
- Members can then digitally share those documents with their family and health care professionals, so the people that know them best, know their care wishes.
- Members are reminded through periodic communications to finalize, share or update these all-important documents and manage them as their life changes.
