More than 150 million people in the United States do NOT have any formal advance directives or critical health care decision documents, (i.e., Medical Power of Attorney, Living Will with Organ Donation, Mental Health and Memorial Service Plans) in case of an emergency and they become incapacitated and cannot make decisions about their health care. Advance directives such as living wills or health care power of attorney forms specify how individuals want medical decisions to be made for them through assigned Agents, should they become unable to communicate their wishes. My Care Voice wants members to have a voice when unplanned or emergency health crises arise.
Yes. A member can create and complete any or all of the 4 directives as part of their membership
Members will be able to create, complete, save and share their documents securely with family, friends and even their physicians through our digital journey program. The most important factors are having the documents easily available and sharing them safely so members voices can be heard in unexpected health care situations, and they are incapacitated.
Our yearly subscription is $29.95. We encourage members to have their family and friends sign-up as well (members must be over the age of 18) to ensure they have their wishes defined and known.

My Care Voice is not a legal service and does not offer legal advice. Our documents are prepared for members to easily print and take to a Notary or their legal counsel for formalizing.
No, our journey allows for members to select which documents are most important to them and complete. Members can also stop at any point and then return to complete when ready.
